            I will remeasure the quilt when I receive it for the final invoice and contact you with any discrepancies.

_initial_____________I acknowledge and agree to the quilting rate of 3 cents per square inch. This includes 80/20 batting, thread, and a new needle. Shipping and other miscellaneous charges are not a part of this fee. I understand that the minimum for quilting services is $50 per project.

• Please mark the top left corner of the top with a safety pin.
• If the back Is pieced together, please denote the top left corner of the back (as it lays in the quilt sandwich) with a safety pin.(The two safety pins will be in the same corner when the quilt sandwich is laid out)
